Program Guidelines

  • Care Club can only be used for care at Lomira Family Dental.
  • This is not an insurance plan, so no need to worry about a deductible or yearly maximum!
  • It cannot be combined with other dental insurance, workers compensation, or accident coverage.
  • Patients must have zero account balance to join.
  • If using Care Club for orthodontics, membership must be maintained for the duration of treatment.
